Beginning at around thirty years of age, testosterone production in men decreases as a result of many causes. How Does it Work?

Bioidentical hormone therapy begins with lab work to determine your body’s unique hormone levels. Then, a mixture of hormones that are targeted to your body’s specific needs is made into a compound in the form of a cream, gel, injection, pellet or capsule. Transdermal, capsule, and injectable forms seem to be best because they bypass the liver and gastrointestinal system. However, the consequence is a rapid increase and rapid decrease in testosterone causing a rollercoaster of side effects as the medication begins to work and stops working before your next dose.

In the beginning of your therapy, you will need frequent testing to check that each hormone component is in proper balance. Once balance is restored, you will only need another treatment in 4-6 months depending on own body’s ability to absorb the pellets. You will normally see improvement within the first two to three weeks of therapy, but it may take up to eight weeks to feel the full effect. In addition, there are various implicated risk factors for SD in men such as altered lifestyle, stressful living conditions, various chronic diseases, diverse environmental pollutants and several drug side effects.

ED often involves a combination of vascular, neurological, endocrinological, and psychological factors, and so the condition is not limited to elderly men. Other risk factors such as cardiovascular disease, hypertension, diabetes, hypercholesterolemia, and smoking have been strongly associated with an increased prevalence of ED.

 

Normally, the blood supply to the penis originates predominantly from the internal pudendal artery, which branches into the penile artery. The penile artery then branches into the cavernous arteries. The cavernous artery enters the cavernous body and subsequently divides into many branches called the helicine arteries, which open into the cavernous sinuses. Blood in the cavernous sinuses is drained by the subtunical veins that form the venous plexuses just beneath the tunica albuginea and then returns to the circulation via 3 sets of veins; the superficial, intermediate and deep veins.

 

In patients with erectile dysfunction, the main issue is insufficient blood flow to the penis. Blood flow is cut off or inadequate due to insufficient opening inside the vessel to allow more blood flow due to plaque build up. This is caused by a variety of factors listed above.

  1. Medications
    1. A class of agents known as type-5 phosphodiesterase (PDE5) inhibitors have become the first-line therapy for ED, as recommended by the American Urological Association (AUA) and the European Association of Urology (EAU). These include oral medications such as Phosphodiesterase-5 (PDE5) inhibitors: Sildenafil (Viagra), tadalafil (Cialis), vardenafil (Levitra), and avanafil (Stendra).
    2. The 3 most commonly used medications for injection therapy are Trimix, Bimix, and papaverine. Most men begin injection therapy with Trimix, which is a mixture of 3 ingredients: alprostadil, phentolamine, and papaverine. These ingredients work by relaxing the smooth muscle and opening the blood vessels in your penis, causing an erection.  Must be stored in the refrigerator. Side effects reported were pain, priapism, bruising, and curvature or the penis.

Is it effective?

What are pellets made of?
Pellets, also referred to as implants, are composed of bio-identical hormones, that is, hormones identical to those our body produces, such as testosterone or estrogen. These hormones are derived from natural plants and are compressed into tiny solid cylinders, larger than a grain of rice but smaller than a ‘Tic Tac.’ Most pellets in the U.S. are produced by compounding pharmacists and delivered in sterile glass vials.

How are pellets different from other hormone therapies?
Pellets are favored because they provide steady, healthy levels of hormones for 4-5 months in men, avoiding the hormonal fluctuations seen with other delivery methods. This stability is crucial for optimal health and disease prevention. Pellets do not increase the risk of blood clots, unlike conventional or synthetic hormone replacement therapy.

In comparison to traditional hormone replacement therapy, pellets have been found superior for relieving menopausal symptoms, maintaining bone density, restoring sleep patterns, and enhancing sex drive, libido, sexual response, and performance. Testosterone delivered via pellet implants has been utilized to treat migraines and menstrual headaches and improve urinary and sexual health. How long will the pellets last?
Pellets generally last 4-5 months in men and do not need to be removed as they dissolve on their own. Not all pellets are created equal, and their efficacy may vary.

How much does this therapy cost?
The cost of pellet insertion varies based on the dose and the number of pellets needed. Men typically require larger doses of testosterone than women and thus may incur a higher cost. The procedure may not be covered by most insurance companies, but pellets often prove cost-effective when compared to the cost of treating the individual symptoms of hormone decline.

What side-effects can I expect?
Allergic reactions to implants are extremely rare, often characterized by a localized area of redness and itching at the implant site. Many patients with such reactions have low cortisol levels and may be recommended cortisol treatment. If itching is bothersome, 25-50 mg of Benadryl may be beneficial.
